HSE Area Finder

The HSE Area Finder allows health care workers from primary care, community care and hospitals to quickly:

•  Associate a patient's Eircode/address with the appropriate Community Healthcare Network (CHN).
•  View that location and CHN boundary on a map.
•  Obtain the contact details for the CHN as follows:
  •    Community Healthcare Network (CHN)
  •    Assistant Director of Public Health Nursing  (ADPHN)
  •    Community Specialist Team for chronic disease (Chronic Disease CST)
  •    Community Specialist Team for older persons (Older Persons CST)
